Make an appointment request below. Generally, initial consultation will be in person at the Holualoa office on a Saturday. For time sensitive/urgent appointment requests, please use the inquiries tab above to provide details of your need and our office will call you promptly - you may qualify for a sooner appointment on a case by case basis.

You will be prompted to provide credit/debit card information when you make an appointment request. This is to enable prompt payment of service after completion of your visit as well as no show fee if incurred. Please cancel or reschedule visits promptly if this becomes necessary in order to avoid this fee. Initial consultations are $350 unless other procedures or ultrasounds are performed which may incur additional cost. If you wish to trade goods/services instead of cash payment please use inquiries tab to discuss this arrangement in advance.
